Here's how you do it:
Create custom questions for attendees to collect more information (like food preference or t-shirt size). To get started, go to Order options and choose Order form. Then click “Add custom question”.
<h2 id="1">1. Go to your Event Dashboard.</h2>
<p dir="ltr">Log into your Eventbrite account. Then select your event.</p>
<h2 id="2">2. Go to "Order Form" (under Order Options).</h2>
<h2 id="3">3. Collect information for the buyer only or for each attendee.</h2>
<p dir="ltr">Choose one of the following:</p>
<ul dir="ltr"><li><b>Buyer Only:</b> Collect information from just the person purchasing tickets.</li><li><b>Each Attendee:</b> Choose this option to collect information from each person attending the event.</li></ul>
<h2 id="4">4. Enable custom questions.</h2>
<ul dir="ltr"><li>Click the <b>Customize Checkout questions</b> toggle.</li><li>Go to the bottom of the existing order form options.</li><li>Click <b>Add custom question</b>.</li></ul>
<h2 id="5">5. Enter your question.</h2>
<p dir="ltr">Click or tap in the field under <b>Question prompt</b> to enter your custom question.</p>
<h2 id="6">6. Choose a question type.</h2>
<ul dir="ltr"><li><b>Text:</b> for short answers to questions like "What speaker would you like to see at a future event?"</li><li><b>Paragraph text:</b> for longer answers to questions like "What do you hope to learn?"</li><li><b>Radio buttons:</b> if you want attendees to only select one option.</li><li><b>Dropdown:</b> if you want attendees to only select one option.</li><li><b>Checkboxes:</b> if you want attendees to be able to choose more than one option.</li><li><b>Additional Terms:</b> use additional terms to collect agreement to the terms and conditions for your event.</li></ul>
<p dir="ltr">If your question type is a radio button, dropdown, or checkbox, you can limit the number of times each answer can be selected. For example, if you limit the number of times an answer can be selected to 20, the answer won’t be available to choose after it’s been selected 20 times. If your event<a href="https://www.eventbrite.com/support/articleredirect?anum=3467" target="_blank"> </a>is recurring, choice limits can't be set for custom questions options.</p>
<h2 id="7">7. Optional: Add conditional sub-questions.</h2>
<p dir="ltr">Check the box labeled <b>Add conditional sub-questions</b> to add a follow up question that depends on the attendee’s answer to the main question (radio button, dropdown, or checkbox only). For example, if you’re serving food at your event, you might have a question that asks if your attendee is vegan or non-vegan. If an attendee selects “vegan” as their answer to your custom question, you might set up a conditional sub-question that asks them to choose from a list of vegan meal options. </p>
<p dir="ltr">Conditional sub-questions can only be asked for the original custom question and not another sub-question.</p>
<h2 id="8">8. Optional: Display the question for specific ticket types.</h2>
<p dir="ltr">If you have ticket types that your custom question doesn’t apply to, select <b>Show this question for specific ticket types</b>. Then choose which tickets you want the question to show for. </p>
<h2 id="9">9. Save your changes.</h2>
<p dir="ltr">The time limit to complete an order is 8 minutes by default, so if you're asking a lot of questions on your order form, you might want to increase the registration time limit.</p>
<p dir="ltr">Attendee answers to custom questions can be found in the Attendee summary report, and also in the Custom question responses report. </p>
